Insurance Information
Insurance cards must be present prior to the office visit. Please notify our office if there is change in insurance plans or coverage. We file claims as a courtesy to our patients. However we do not become involved in any disputes between insurance and the patient. Any dispute for unpaid charges will be billed to you. All patients must have an insurance card in order to utilize benefits. It is the patients responsibility to disclose insurance information (primary and secondary plans). If your insurance requires a referral it will be your responsibility to contact your PCP to make sure one has been issued.
